Recently, the People's Government of Gantai County, Gansu Province, issued the "Key Points of Medical Security Work in 2021", which clearly states the implementation of the "one invoice system" for medical consumables!
The key points propose: to continue deepening the reform of the drug and medical consumables procurement system, improve the classification procurement policy for drugs and medical consumables, gradually open up the distribution market, encourage and support production enterprises to deliver directly or designate delivery enterprises, vigorously promote the "one invoice system" delivery, and promote the return of drug and high-value consumable prices to a reasonable level.
Strengthen monitoring and early warning of abnormal situations in drug and medical consumable prices and supply, ensuring clinical use in medical institutions. Implement the drug price and procurement credit evaluation system, creating a fair, standardized, and clean circulation order and trading environment.
Many provinces have implemented it, and the "one invoice system" is accelerating.
From two invoices to one invoice, it seems to happen in an instant. In 2018, the former National Health and Family Planning Commission issued the "Notice on Consolidating the Achievements of Breaking the Practice of Using Medicine to Supplement Medical Services and Continuously Deepening the Comprehensive Reform of Public Hospitals", officially rolling out the "two invoice system" for high-value medical consumables nationwide.
Unexpectedly, at the same time as the "two invoice system" was implemented, many provinces have begun to simultaneously promote the "one invoice system" for consumables. Taking Jiangsu Province as an example, it issued the "Notice on Promoting Sunshine Procurement of High-Value Medical Consumables in the Province", guiding and promoting unified settlement of procurement costs for public medical institutions in the province and achieving integration of business flow, information flow, and capital flow on the provincial platform. It is widely believed in the industry that the main purpose of the integration of the three flows is to implement the "one invoice system".
Provinces such as Shandong, Fujian, Hunan, Guangdong, and Zhejiang have also issued response documents, and while the "two invoice system" is being implemented, they are piloting and encouraging the implementation of the "one invoice system" for medical consumables!
Recently, in addition to Gantai County, Gansu, the city of Pingdingshan in Henan Province, in the issued "Notice on Further Strengthening the Management of Drug and Consumable Distribution in Public Medical Institutions", clearly states: prioritize choosing integrated drug and consumable enterprises to implement the one invoice system, directly delivering drugs and consumables to our city's medical and health institutions.
In the public bidding project for the selection of suppliers for intravenous catheter (pack) consumables at the Second Affiliated Hospital of Xi'an Jiaotong University, it is directly required: suppliers must provide a letter of commitment for all products bid to meet the requirements of the one invoice system or two invoice system.
Distributors and delivery companies are accelerating their exit.
In fact, there has always been much controversy in the industry regarding the "one invoice system". Some believe that the price of drugs and devices has nothing to do with the number of invoices, while others believe that such practices violate market development laws.
Because, unlike the "two invoice system", under the "one invoice system", not only is the excess in the circulation link "squeezed out", but the agency distribution model is also directly canceled, making it impossible for intermediaries to profit from the price difference, leading to the exit of a large number of distributors!
At the same time, under the prevalence of the SPD management model for medical consumables, large pharmaceutical distribution giants such as Shanghai Pharmaceuticals and China National Pharmaceutical Group monopolize the delivery rights of consumables for many top-tier and second-tier hospitals, accelerating the exit of small and medium-sized distributors. This model has already sunk from secondary and tertiary hospitals to county hospitals!
Recently (April 22), the China Government Procurement Network published the "Bid Announcement for the Procurement of Medical Equipment and Consumables for the Laboratory Department of the New Hospital Area of Changle County People's Hospital", which states that the procurement and delivery project (9.365 million) was exclusively won by Shandong Haiwang Galaxy Pharmaceutical Co., Ltd., which is reportedly one of the largest pharmaceutical distribution companies in Shandong Province.
Bundled delivery requires higher demands and poses greater challenges for companies, while most small and medium-sized distribution companies do not possess such competitive strength. Undoubtedly, under the new procurement and delivery model, large enterprises are continuously expanding their territory, while small and medium-sized enterprises and distributors seem to be accelerating their elimination.
